Waiver and Release

I, the parent/guardian of the above named child, do hereby give my permission and approval for his/her participation in Devils Lacrosse Club activities, including related transportation and assume all such risks and hazards incidental to such participation. I do hereby, waive, release, absolve, indemnify and agree to hold harmless Devils Lacrosse Club, including officers, sponsors, managers, coaches and other participants (hereafter the Releases) with respect to any injury, damage, or loss sustained by my child whether the result of negligence on the part of any or all of the Releases or due to any other cause.

Also, I agree to return the uniform and other equipment issued to my child in as good condition as received, except for normal wear and tear.

I further hereby acknowledge that this waiver and release is given voluntarily and with the complete understanding and knowledge that my child's participation in Devils Lacrosse Club is being permitted by me based on my full understanding of the limitation of rights imposed by my execution of the Parental Release and with full knowledge of the risks of serious injury that my child is exposed to by his/her participation in Devils Lacrosse Club.
